Application criteria: (Applicants must fulfill all of the followings)
1. Holder of Hong Kong ID card or valid travel document; and
2. Patients diagnosed and prescribed with Ebglyss® by a registered doctor of Hong Kong hospital or clinic within past 6 months
Remarks: No income or financial assessment is required
