♦ Promote general awareness and knowledge about the correct and safe use of medications;

♦ influence individuals behavior towards the correct and safe use of medications;

♦ enhance one's ability to manage the daily operational needs on one's or his/her family members' or others' needs on correct and safe use of medications;

♦ enable more community elderly centers to take on the long term need of fulfilling the drug knowledge gap and drug management principles;

♦ provide a means and assist the society at large to better manage an increasingly important aspect of needs in life.

服務內容： 

1.      為社區長者提供藥物管理服務：65歲或以上患有長期慢性疾病的長者，而又需要服用多種藥物，可以透過外展藥劑師一對一的咨詢服務，建立個人化的藥物管理，讓長者加深對自己所服藥物的了解，更會為長者簡化及便利化服藥的程序。 
 
2.      藥物知識及管理教育培訓：60至69歲的年輕長者，可報名参加主題藥物講座或基礎藥物管理證書課程的培訓，掌握基本用藥知識，透過學習如何運用流動應用程式及正確的網上健康資訊，達到自我管理健康; 完成課程學員更有機會成為藥物大使，傳播正確的藥物知識，幫助有需要的家人及身邊的朋輩！
