Application criteria: (must fulfill all of the followings)

1. Valid HKID card holder

2. Prescribed with Zejula by same private oncologist, gynaecological oncologist or gynaecologist in Hong Kong

3. Adult patients who meet the medical conditions specified by this program

4. Financial status: Individual monthly income not exceeding HK$ 80,000 in total

5. Not enrolled in other Zejula patient assistance programs
