1. Valid HKID card holder

2.  Patients who meet the medical conditions specified by this program

3.  Passed income assessment  with individual monthly income not exceeding HK$ 80,000 in total

4. With Zejula® prescription prescribed by same private oncologist or gynaecologist

5. Have not joined any other Zejula® drug assistance program
