SKYRIZI Patient Support Program
Application criteria: (Applicant must fulfil ALL 5 of the following criteria)
1. Patient is a Hong Kong citizen and has a valid HKID card;
2. Patient is an adult and an existing patient with a private dermatologist or rheumatologist clinic in Hong Kong;
3. Patient either is an existing patient at, has an appointment with, or has a referral letter to the Hospital Authority or Department of Health hospital/clinic;
4. Patient has an annual (past 12 months) household income of HK$2,000,000 or less;
5. The cost of the patient’s Skyrizi treatment is not and will not be covered by any insurance policies where the patient is a beneficiary.
