Application criteria
1. Valid HKID card holder
2. Adult patients of any private hospitals or private specialist clinics in Hong Kong
3. Meet the medical conditions specified by this program
4. Financial status: Personal monthly income not exceeding HK$ 100,000 in total
5. Having the receipt or purchasing proof and original prescription of Lynparza™ issued by any Hong Kong private clinical oncologist or medical oncologist
6. If applicant has already enrolled in any other Lynparza™ drug assistance program, applicant can only apply this program with prior approval only
