Application criteria
1. Hong Kong residents: Valid HKID card holder; Non-Hong Kong residents: Valid passport or identity card issued from residing country and
2. Adult patients of any private oncology or haematology and haematological oncology centre or private hospital in Hong Kong, and
3.Having original prescription of Brukinsa® issued by any Hong Kong doctor of private haematology and haematological oncology, clinical oncology or medical oncology, and
4. Having the purchase invoices, receipts and purchase record of Brukinsa® purchased in Hong Kong, and
5. Patient, who has already enrolled in any other Brukinsa® drug assistance program, cannot apply for this program
