Applicant Requirements
1. Valid Hong Kong permanent resident identity card holder
2. Patients from private oncology centres or private hospitals in Hong Kong
3. Female patients with Ibrance® prescription issued by Hong Kong oncologists, and meet the medical conditions specified by this program
4. Holding receipts or purchasing record booklet of Ibrance® issued by private practice oncologists
5. Successful applicants of other Ibrance® subsidy schemes are not eligible for this program
Free Drug Collection Procedure
1. Register in person at SafeMed Dispensary at appointment time with the following documents:
♦ Referral letter from private practice oncologists
♦ Original Hong Kong identity card
♦ Receipts (original or certified true copy) or purchasing record booklet of Ibrance® verified by private oncologists
2. During the first 18 treatment cycles (self-financed):
♦ Attend at least 4 complimentary and mandatory medication consultation sessions conducted by pharmacists
♦ Submit receipts (original or certified true copy) or purchasing record booklet of Ibrance® during medication consultation
3. After the first 18 treatment cycles, collect free drug at SafeMed Dispensary in person at appointment time with the following documents:
♦ Named receipts (original or certified true copy) or a purchasing record booklet of Ibrance® (for the first time of complimentary drug collection)
♦ Verification letter from oncologists (for the first time of complimentary drug collection)
♦ Original copy of valid doctor prescription from private practice oncologists
♦ Original Hong Kong identity card
4. Pay administration and service fee and receive counseling from pharmacy before collecting free drug.
