Enzalutamide (Xtandi®)

Program Period:

From 15th March 2019 until further notice

Applicants’ Criteria:

    1. A Valid Hong Kong Identity Card (HKID) Holder

    2. A Patient from Private Practice Urologists or Oncologists Registered in Hong Kong

    3. Disease Status：

        • Castration-Resistant Prostate Cancer

        • Confirmed Metastases

    4. Financial Status: Individual Annual Disposable Income less than HKD 1.5 million

    5. Hold valid prescriptions and named receipts (original or certified true copy) or a purchase record booklet of Xtandi® verified by your private practice urologists and oncologists

    6. Applicants should not apply and enroll into other capping and subsidy programs of Xtandi® if he is successfully enrolled into Xtandi® private capping program under SafeMed Dispensary of HKPCF
